ABSTRACT

Objective: To analyze the short-time efficacy of empagliflozin in the treatment of glycogen storage disease type Ⅰb (GSD Ⅰb). Methods: In this prospective open-label single-arm study, the data of 4 patients were collected from the pediatric department in Peking Union Medical College Hospital from December 2020 to December 2022. All of them were diagnosed by gene sequencing and had neutropenia. These patients received empagliflozin treatment. Their clinical symptoms such as height and weight increase, abdominal pain, diarrhea, oral ulcer, infection times, and drug applications were recorded at 2 weeks, 1 month, 2 months, 3 months, 6 months, 9 months, 12 months, and 15 months after treatment to assess the therapeutic effect. The liquid chromatography-tandem mass spectrometry method was used to monitor the changes in 1, 5-anhydroglucitol (1, 5AG) concentration in plasma. At the same time, adverse reactions such as hypoglycemia and urinary tract infection were closely followed up and monitored. Results: The 4 patients with GSD Ⅰb were 15, 14, 4 and 14 years old, respectively at the beginning of empagliflozin treatment, and were followed up for 15, 15, 12 and 6 months, respectively. Maintenance dose range of empagliflozin was 0.24-0.39 mg/(kg·d). The frequency of diarrhea and abdominal pain decreased in cases 2, 3, and 4 at 1, 2 and 3 months of treatment, respectively. Their height and weight increased at different degrees.The absolute count of neutrophils increased from 0.84×109, 0.50×109, 0.48×109, 0.48×109/L to 1.48×109, 3.04×109, 1.10×109, 0.73×109/L, respectively. Granulocyte colony-stimulating factor was gradually reduced in 1 patients and stopped in 3 patient. Plasma 1, 5 AG levels in 2 children were significantly decreased after administration of empagliflozin (from 46.3 mg/L to 9.6 mg/L in case 2, and from 56.1 mg/L to 15.0 mg/L in case 3). All 4 patients had no adverse reactions such as hypoglycemia, abnormal liver or kidney function, or urinary system infection. Conclusion: In short-term observation, empagliflozin can improve the symptoms of GSD Ⅰb oral ulcers, abdominal pain, diarrhea, and recurrent infection, also can alleviate neutropenia and decrease 1, 5AG concentration in plasma, with favorable safety.

ABSTRACT

AIM: To investigate the occurrence and possible mechanism of blue light-induced ferroptosis in retinal pigment epithelial cells.METHODS: ARPE-19 cells cultured in vitro were irradiated by 405 nm blue light at 50 mW/cm2 irradiance with different duration and were divided into control, 16.3J/cm2, 32.6J/cm2, and 65.2J/cm2 groups; the 65.2J/cm2 group was defined as the high-level blue light irradiation group and cells were further divided into control, high-level blue light irradiation group and high-level blue light irradiation + ferroptosis inhibitor group. CCK-8 assay was used to detect cell viability, commercial kits were used to detect intracellular glutathione(GSH), ferrous iron and malondialdehyde(MDA)concentration, and Western blot was used to detect the relative expression of glutathione peroxidase 4(GPX4)and xCT proteins in cells.RESULTS: The decrease of ARPE-19 cell viability caused by blue light irradiation was dose-dependent, and the reduction of intracellular GSH concentration, the increase of ferrous iron concentration and MDA concentration were all caused by high-level blue light irradiation(all P<0.05); the ferroptosis inhibitor partially restored cell viability and recovered intracellular GSH, reduced concentrations of MDA and ferrous iron in the blue light irradiation group(all P<0.05). The relative expressions of GPX4 and xCT proteins were significantly decreased in the blue light irradiation group, and such change was alleviated by the ferroptosis inhibitor(P<0.05).CONCLUSION: Blue light irradiation may induce ferroptosis in RPE cells by targeting the xCT and GPX4-associated antioxidant pathways.

ObjectiveTo investigate the level of serum antibodies in COVID-19 patients six months after discharge, and to provide data to evaluate the duration of IgM, IgG and neutralizing antibody titers in the patients. MethodsEnzyme-linked immunosorbent assay (ELISA) was used to determine the antibody levels of IgM and IgG, and the new coronavirus live virus neutralization test was used to detect the neutralizing antibodies in the plasma of 181 recovered patients. ResultsThe IgG positive rate was 92.27% (167/181) in COVID-19 patients six months after discharge, while the lgM positive rate was 28.18% (51/181). Six months after hospital discharge, 117 recovered patients (64.64%) were positive for IgG antibodies and negative for IgM antibodies, indicating that they had produced stable antibodies. This result suggested that they had been infected with the new coronavirus (SARS-CoV-2) and were in the recovery stage. The positive detection rate of neutralizing antibodies was as high as 91.71%. ConclusionSix months after infection with SARS-CoV-2, IgG antibodies produced in the patients continue to exist, and the neutralizing antibodies maintain a high and stable level. Results of this study have important guiding significance for future research on the durability of new coronavirus antibodies.

ABSTRACT

The present study aims to investigate the effects of the main components(aesculin, berberine hydrochloride, and anemoside B4) in the butyl alcohol extract of Baitouweng Decoction(BAEB) on the chemotaxis of neutrophils induced by dimethyl sulfoxide(DMSO). HL60 cells were cultivated in RPMI-1640 complete medium, and transferred into a 6-well plate(2 × 10~5 per mL) with 4 mL in each well, followed by incubation with DMSO at 1.3% for five days. The morphologic changes of cells were observed under an inverted microscope. The CD11 b expression after DMSO induction was analyzed by flow cytometry. The effects of aesculin, berberine hydrochloride, and anemoside B4 on the cell proliferation and migration were detected by CCK8 assay and Transwell assay, respectively. The effects of the main components on the production and polarization of F-actin protein were also examined by flow cytometry and laser confocal microscopy. PI3 K/Akt signaling pathway was checked by Western blot. As revealed by the results, neutrophil-like HL60 cells were observed after DMSO induction. The CD11 b expression in these cells increased significantly as indicated by the flow cytometry. Additionally, 100 μg·mL~(-1) aesculin, 8 μg·mL~(-1) berberine hydrochloride, and 80 μg·mL~(-1) anemoside B4 were potent in inhibiting the migration of neutrophils and reducing F-actin expression. Berberine hydrochloride was verified to be capable of diminishing phosphorylated PI3 K/Akt protein expression. The findings indicate that aesculin, anemoside B4, and especially berberine hydrochloride in the BAEB can inhibit the chemotaxis of neutrophils, which is possibly achieved by the inhibition of F-actin and PI3 K/Akt signaling pathway.

ABSTRACT

Objective To understand the population distribution, density, seasonal fluctuation and nocturnal activity of malaria vectors in Anhui Province from 2016 to 2018, so as to provide a data support for formulating the control strategy for imported malaria during the malaria post-elimination stage. Methods The malaria vectors were monitored in 105 counties (cities or districts) of Anhui Province from 2016 to 2018, and the population density, seasonal fluctuation and nocturnal activity of the mosquitoes were observed using the lamp trapping and human bait trapping methods. The density of Anopheles mosquitoes was compared among different years, regions and mosquito-capturing sites. Results Anopheles mosquitoes were captured in 103 counties (cities or districts) of Anhui Province during the period from 2016 to 2018, and a total of 32 494 mosquitoes were captured using the lamp trapping method and 36 228 captured using the human bait trapping method. All captured mosquitoes were morphologically identified as Anopheles sinensis, and no An. anthropophagus was found. The density of An. sinensis peaked from June to August, and the peak nocturnal activity was found during the period between 19∶00 and 23∶00. Among all mosquito-capturing sites, the highest mosquito density was seen in the livestock and poultry sheds (H = 18.835, P < 0.05). The density of An. sinensis varied significantly in regions in 2016 and 2017 (H = 16.655 and 11.566, P < 0.01), and a low density was found in north of the Huai River. Conclusions An. sinensis is widely distributed in Anhui Province, which is the currently predominant malaria vector in the province. During the malaria post-elimination stage, the malaria vector monitoring should be intensified and vector control interventions should be timely adopted in epidemic foci of Anhui Province to prevent the local re-transmission of overseas imported malaria.

ABSTRACT

Objective To analyze the epidemiological characteristics of imported malaria cases and investigate the influenc-ing factors of severe cases in Anhui Province,so as to provide the evidence for improving the control strategy. Methods The ep-idemiological data of imported malaria cases in Anhui Province from January 1st,2012 to April 18th,2017 were collected and analyzed. Results The imported malaria cases in Anhui Province were mainly young men of migrant workers. There were im-ported malaria cases in 79%(83/105)of counties(districts)in Anhui Province. . Totally 686 imported malaria cases were re-corded,in which there were 62 severe cases(9.04%),and four death cases with the mortality rate of 0.58%. The age,educa-tion,initial diagnosed departments,initial diagnosed results,time from onset to seeing a doctor,and time from seeing a doctor to diagnosis were the influencing factors for the severe cases. Conclusion The proportion of severe imported malaria cases is high in Anhui Province. The monitoring sensitivity,clinician's awareness of malaria diagnosis,and health education for migrant workers should be improved.

ABSTRACT

Objective To investigate the clinical improving effect on the symptoms of acupuncture at point Weizhong(BL40) plus following lumbar vertebra-stabilizing core muscle group training in patients with lumbar intervertebral disc herniation. Method Sixty patients with lumbar intervertebral disc herniation were randomized to a treatment group (30 cases) and a control group (30 cases). The treatment group received acupuncture at point Weizhong plus following lumbar vertebra-stabilizing core muscle group training and the control group, oral administration of celecoxib capsules. After treatment, the clinical improving effect on the symptoms was evaluated in the patients using the NRS, JOA Lumbar Scoring and ODI. Result The total efficacy rate was 100% in the treatment group and 53.3%in the control group. Post-treatment NRS, JOA and ODI scores were better in the treatment group than in the control group; there were statistically significant differences (P<0.001). Conclusion Acupuncture at point Weizhong plus following lumbar vertebra-stabilizing core muscle group training can effectively treat lumbar intervertebral disc herniation.

ABSTRACT

Nonketotic hyperglycinemia (NKH) is an autosomal recessive hereditary disease caused by a defect in the glycine cleavage system and is classified into typical and atypical NKH. Atypical NKH has complex manifestations and is difficult to diagnose in clinical practice. This article reports a family of NKH. The parents had normal phenotypes, and the older brother and the younger sister developed this disease in the neonatal period. The older brother manifested as intractable epilepsy, severe spastic diplegia, intellectual disability, an increased level of glycine in blood and cerebrospinal fluid, an increased glycine/creatinine ratio in urine, and an increased ratio of glycine concentration in cerebrospinal fluid and blood. The younger sister manifested as delayed language development, ataxia, chorea, mental and behavior disorders induced by pyrexia, hypotonia, an increased level of glycine in cerebrospinal fluid, and an increased ratio of glycine concentration in cerebrospinal fluid and blood. High-throughput sequencing found a maternal missense mutation, c.3006C>G (p.C1002W), and a paternal nonsense mutation, c.1256C>G (p.S419X), in the GLDC gene in both patients. These two mutations were thought to be pathogenic mutations by a biological software. H293T cells transfected with these two mutants of the GLDC gene had a down-regulated activity of glycine decarboxylase. NKH has various phenotypes, and high-throughput sequencing helps to make a confirmed diagnosis. Atypical NKH is associated with the downregulated activity of glycine decarboxylase caused by gene mutations.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical features, laboratory findings, diagnosis and treatment, and prognosis of children with systemic lupus erythematosus (SLE) accompanied by pulmonary hypertension (PH).</p><p><b>METHODS</b>The clinical symptoms, laboratory findings, echocardiographic features, SLE disease activity index, and treatment outcome of 15 hospitalized children with SLE accompanied by PH were retrospectively analyzed.</p><p><b>RESULTS</b>Among the 15 patients, the median interval from diagnosis of SLE to diagnosis of PH was 0.1 year (range: 0-6.5 years). Aside from PH-related symptoms, Raynaud's phenomenon was observed in 6 (40%) of the 15 patients. There was no significant difference in SLE disease activity (evaluated by complements 3 and 4 levels, erythrocyte sedimentation rate, and positive rate of anti-double-stranded DNA) between patients with mild-to-moderate PH and those with severe PH (P<0.05). As for treatment, 13 patients received immunosuppressive therapy with glucocorticoids, and among them 2 patients received PH-targeted therapy. During a median follow-up of 8.0 years (range: 0.5-18.1 years) since the diagnosis of PH, 2 deaths were noted with class III or IV cardiac function (World Health Organization), while the other patients were in a stable condition.</p><p><b>CONCLUSIONS</b>Raynaud's phenomenon is a common clinical manifestation in children with SLE accompanied by pulmonary hypertension (PH). PH severity is not significantly associated with SLE disease activity, and thus greater focus should be placed upon early screening of pulmonary arterial pressure in SLE patients. Early diagnosis and early treatment can improve the prognosis of children with SLE.</p>

ABSTRACT

<p><b>BACKGROUND</b>The clinical significance of metabolic syndrome (MS) score, MS, and its individual components with respect to risk prediction of coronary artery disease (CAD) remains unclear. The objective of this study was to investigate whether and to what extent MS score, MS, and its individual components were related to the risk of CAD.</p><p><b>METHODS</b>Among 1191 participants who underwent coronary angiography for the confirmation of suspected myocardial ischemia, 858 were included in this study according to the inclusion criteria from September 2010 to June 2013. MS was diagnosed with the 2005 National Cholesterol Education Program Adult Treatment Panel III criteria. The severity of coronary atherosclerosis was assessed by Gensini score.</p><p><b>RESULTS</b>The results showed that the age- and sex-adjusted odds ratios (OR s) for CAD were as follows: MS score, 1.327; MS, 2.013; elevated waist circumference, 1.447; reduced high-density lipoprotein cholesterol, 1.654; and elevated fasting glucose, 1.782; all P < 0.05; whereas for elevated triglycerides, 1.324, and elevated blood pressure, 1.342, both P > 0.05. After multivariate adjustment, results showed that only MS and elevated fasting glucose were significantly associated with CAD (OR, 1.628, 95% confidence interval [CI], 1.151-2.305, P = 0.006 for elevated fasting glucose, and OR, 1.631, 95% CI, 1.208-2.203, P = 0.001 for MS). The study showed that only MS score and elevated fasting glucose were significantly associated with Gensini score (standardized coefficient, 0.101, P = 0.031 for elevated fasting glucose and standardized coefficient, 0.103, P = 0.009 for MS score).</p><p><b>CONCLUSIONS</b>The present study demonstrated that MS score, MS, and its individual components might have different contributions to CAD prevalence and severity. MS and elevated fasting glucose were independent risk factors for the prevalence of angiographic CAD whereas MS score and elevated fasting glucose were significantly associated with the severity of CAD.</p>

ABSTRACT

<p><b>BACKGROUND</b>Steroid 11beta-hydroxylase deficiency (11beta-OHD), an autosomal recessive inherited disease, accounts for 5% - 8% of congenital adrenal hyperplasia. It was scarcely reported in China. This article reports two Chinese girls with 11beta-OHD.</p><p><b>METHODS</b>The two patients were sisters and presented with hypertrichosis, skin pigmentation, laryngeal prominence and virilization of external genitalia. The patients were followed up for their clinical symptoms and signs, hormone profile, and adrenal image. The genomic deoxyribonucleic acids of the patients and their parents were isolated. 11beta-hydroxylase gene (CYP11B1) was amplified by polymerase chain reaction and directly sequenced.</p><p><b>RESULTS</b>Hormone tests showed that serum cortisol was in the low limit of normal range, whereas the concentrations of adrenocorticotropic hormone, testosterone and progesterone were much higher than those of normal adult females. There were obvious adrenal hyperplasia and advance of bone age. After 11 months of treatment with dexamethasone, the skin pigment became regressed; the breast, uterus and ovary gradually developed and normal menstrual cycle started while the manifestations of virilization did not change. A single point mutation of CYP11B1 (R454C, GGC --> TGC) in all the members of this family was detected. The sisters were homozygous and their parents were heterozygous.</p><p><b>CONCLUSIONS</b>The clinical manifestation of 11beta-OHD is complicated. The manifestation of virilization could not regress after treatment with dexamethasone. The novel missense mutation of CYP11B1 (R454C, GGC --> TGC) is the pathogenesis of 11beta-OHD at least in some Chinese patients.</p>

ABSTRACT

Objective: To summarize our experience on perioperative anesthesia management of patients with crush syndrome caused by earthquake under field condition. Methods: The clinical anesthesia data of 10 patients with crush syndrome caused by China Wenchuan earthquake, who were treated under field condition, were retrospectively analyzed, and our experience on perioperative anesthesia management was summarized. Results: After volume therapy, urine alkalization, and stabilization of internal environment, the 10 patients underwent debridement, decompression by fasciotomy or amputation under epidural anesthesia or brachial plexus block plus psychological therapy and moderate tranquillizer. During the operation, the patients showed smooth respiratory and cardiovascular characteristics without pain. Conclusion: Under field condition, regional anesthesia can provide adequate analgesic effect during and after the surgery. Early fluid resuscitation and life support are vital to the safety and life of patients.

OBJECTIVE@#To investigate the effect of regulation of VIPhybrid, an unselective antagonist of vasoactive intestinal peptide receptors (VIPR), on the formation and development of form deprivation myopia (FDM) in chick and the expression of protein and mRNA of VIP on the retina and choroids of in chicks.@*METHODS@#Seventy-two 1-day-old yellow healthy leghorn chicks were assigned into 6 groups (12 in each group). Eyes in Group I were covered on the right as a blank control group. Eyes in GroupII were those eyes having been injected with 20 microL saline into vitreous cavity and then covered as a negative control group. Eyes in GroupIII,IV and V were injected with 20 microL VIPhybrid with low (3*10(-12) mol/L), middle (3*10(-10) mol/L) and high (3*10(-8) mol/L) dosage into vitreous cavity and then covered as experimental groups. The above groups had been continuously covered for 1 week. Eyes in Group VI were uncovered and uninjected as a normal control group. Diopter was detected using retinoscopic refraction. The eyeball axis was determined using ophthalmological ultra-A. The expression of protein and mRNA of VIP on retina-choroids-sclera were investigated by SP immunohistochemistry staining and RT-PCR.@*RESULTS@#Form deprivation for 1 week induced high myopia eyes and elongated eyeball axis in GroupI and GroupII, and there was no difference between the 2 groups (P>0.05). The diopter and eyeball axis were significantly reduced in Group III, IV, and V as compared with Group I and II (P<0.01), but the diopter was higher and the eyeball axis was longer than those of Group VI. The diopter and eyeball axis had negative correlation with the concentration gradient of VIPhybrid. The expressions of protein and mRNA of VIP in Group III, IV, and V were down-regulated as compared with those of Group I and I I(P<0.01)and also down-regulated with the increase of concentration of VIPhybrid.@*CONCLUSION@#VIPhybrid can decrease the development of FDM in chicks, which may provide a new pathway for drug therapy of myopia in human beings.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of inhaled nitric oxide (NO) on surfactant protein A (SP-A) and mannose binding ability (MBA) in neonatal rats with hyperoxia-induced lung injury.</p><p><b>METHODS</b>Sixty-four neonatal rats were randomly exposed to room air (Control group), >95% oxygen for 6 days (Hyperoxia group), 10 ppm NO for 24 hrs (NO group), and >95% oxygen for 6 days along with 10 ppm NO for 24 hrs (Hyperoxia + NO group). After 2 and 6 days of exposure, the lung pathologic changes, gene and protein expressions of SP-A and MBA were measured.</p><p><b>RESULTS</b>The rats from the Hyperoxia group presented with obvious lung injuries. The SP-A expressions of mRNA (0.81 +/- 0.04 vs 1.53 +/- 0.25) and protein (59.45 +/- 18.37 vs 89.77 +/- 16.41) in the Hyperoxia group decreased significantly 2 days after exposure but increased significantly 6 days after exposure (SP-A mRNA 0.81 +/- 0.02 vs 0.63 +/- 0.03; SP-A protein 93.57 +/- 13.71 vs 47.73 +/- 21.69) compared with those of the Control group (P < 0.05). NO treatment alleviated the hyperoxia-induced pathologic injuries 2 days after exposure. The SP-A mRNA expression (0.55 +/- 0.91) in the Hyperoxia + NO group was significantly reduced as compared to both the Control and Hyperoxia groups (P < 0.05), and the SP-A protein expression (55.12 +/- 17.53) in the Hyperoxia + NO group was noticeably lower than that of the Control group (P < 0.01) 2 days after exposure. The SP-A protein expression in the Hyperoxia + NO group (67.33 +/- 18.59) was significantly lower than that of the Hyperoxia group 6 days after exposure (P < 0.05). Two days after exposure, the NO group had significantly higher MBA than the Control group (0.821 +/- 0.133 vs 0.58 +/- 0.158); the Hyperoxia + NO group had significantly higher MBA than the Hyperoxia group (0.43 +/- 0.175 vs 0.738 +/- 0.141) (P < 0.05).</p><p><b>CONCLUSIONS</b>Inhaled low dose NO may decrease SP-A protein expression and increase MBA of the lung tissue. This lessens the pathologic lung injury in neonatal rats with hyperoxia.</p>

ABSTRACT

OBJECTIVE@#To investigate the dynamic expression and significance of vasoactive intestinal peptide receptor 2 (VIPR2) on retina-choroid-clera in high myopia.@*METHODS@#Twenty-one yellow chicks of 1 day old were used in the research. The right eyes were the experimental group, covered continuously for 1 week, 2 weeks and 4 weeks respectively. The left eyes were not covered as the normal control group. Both groups were detected diopter degrees using retinoscopic refraction, determinated eyeball axis using ophthalmology ultra-A, and investigated VIPR2 expression on retina-choroid-sclera in both groups at three stages by SP immunohistochemical staining.@*RESULTS@#The experimental eyes changed from hypermetropia at pre-experiment to high myopia during the experiment stages, and the diopter degrees were deeper and eyeball axis was longer along with the period of being covered. Both groups had strong expression of VIPR2 on photoreceptor-outer segment of the retina and choroids. The expression was down-regulated with the time in both groups. Compared with the control group, VIPR2 expression of the experimental group was significantly up-regulated (P < 0.05).@*CONCLUSION@#Form deprivation could induce high myopia. The expression of VIPR2 existed on photoreceptor-outer segment of the retina and choroids. VIPR2 may play an important role on the formation and development of myopia.

ABSTRACT

Objective:To summarize our experience on perioperative anesthesia management of patients with crush syndrome caused by earthquake under field condition.Methods:The clinical anesthesia data of 10 patients with crush syndrome caused by China Wenchuan earthquake,who were treated under field condition,were retrospectively analyzed,and our experience on perioperative anesthesia management was summarized.Results:After volume therapy,urine alkalization,and stabilization of internal environment,the 10 patients underwent dehridement,decompression by fasciotomy or amputation under epidural anesthesia or brachial plexus block plus psychological therapy and moderate tranquillizer.During the operation,the patients showed smooth respiratory and cardiovascular characteristics without pain.Conclusion:Under field condition,regional anesthesia can provide adequate analgesic effect during and after the surgery.Early fluid resuscitation and life support are vital to the safety and life of patients.

ABSTRACT

The present study demonstrated that bone marrow cells from diabetic rats were able to form cell clusters expressing insulin,C-peptide,glucagon,somatostatin and islet amyloid .polypeptide,and other genes associated with development and function of islets such as glucose transporter-2,glucokinase,glucagon-like peptide-1 receptor,PDX-1,Ngn3,NeuroDl,Pax-6 and NKX2.2 genes.These islet-like cells might be derived from adult stem cells in bone marrow.
